Documentation gap checks for NDIS providers

What it is

Gap Check reviews your organisation-level documentation — policies, procedures, registers and training records — against the NDIS Practice Standards modules that apply to your registration groups. You receive a written report with each gap rated and a prioritised closure plan, at a fixed price agreed before work begins.

That includes the SIL module (Module 5A), in force since 1 July 2026, and the Core Module requirement for a documented program of internal audits proportionate to your size and complexity.

Scope

Organisation-level documentation only. Participant files, clinical records and individual worker performance are not reviewed. Gap Check is preparation support — it is not an audit, certification, or legal advice. We are not an approved quality auditor, we do not conduct audits, and we make no claims about the outcome of any audit.

Fee

A fixed fee, set by the audit pathway your registration groups place you in, taken from the public NDIS register rather than from an estimate. You get the figure in writing before anything starts, and it does not move afterwards. Half on engagement, half on delivery. One re-check of the same documents is included at no charge within 30 days.

The new SIL Practice Standards

The SIL Practice Standards changed on 1 July 2026. If you are registered for supported independent living, Module 5A is what you will be assessed against at your next audit — mid-term or renewal, not only at initial registration.

We have written a one-page summary of the four outcomes and which of your documents each one touches. Email us and we will send it — no cost, and no follow-up unless you ask for one.

How your documents are handled

We never accept participant information. Participant files, clinical records and health information are out of scope, are not requested, and are not accepted — if any reaches us in error we delete it and tell you in writing.

Only organisation-level documents are reviewed. Delivering that work uses third-party services for email, file storage and AI-assisted document review, and some of those providers may store or process data outside Australia. Our privacy policy sets this out in full.

Gap Check is run by Ahmed Ademoglu, a registered nurse with clinical governance experience, based in Queensland. Sole trader, ABN 24 289 004 542.
